Our job to look after your health information and keep it private. To best of our ability,  we follow the guidelines of sharing information in the Canadian Standards Association Model Code for the Protection of Personal Information and Personal Health Information Protection Act (PHIPA, 2004).

Where is your information stored?

We store your Personal Health Information digitally in a centralized database system. This tool ensures that your information is accurate and safely stored.

How is the information used?

We use and share your Personal Health Information with external services and health care providers as permitted by law, as well as within your circle of care at Surrey Place to support the services you are receiving.

 Depending on the situation, your personal health information may also be used to receive payments for services and for teaching opportunities.

What are your rights?

Access 
Your Personal Health Information belongs to you. You have the right to request access to your information and a list of services or providers who received your information.

Correction 
 You can request to correct or complete your information if you think it is incorrect or incomplete.

Confidentiality 
We have an obligation to protect your information from going missing, being stolen, and others sharing or misusing it. You can also withdraw your permission, and we will stop using and sharing your information.

Cookies and Website Data Collection

Similar to many websites, our website uses and collects digital “cookies” to gather general trend data concerning traffic on the website for statistical analysis. A “cookie” is a small piece of computer code that collects data while you browse. Cookies by themselves do not tell us your email address or otherwise identify you personally. You can set your web browser to accept or reject cookies.

We also track general information on visits to our website to better understand and improve the information users are accessing. This information is used for to improve the content and experience of our website. Website users will not be personally identified through this data, and Surrey Place does not see any data or contact information on an individual level.
